Call A Doctor Plus Group Enrollments

In order to enroll your group in Call A Doctor Plus, please first create a spreadsheet of all your employees with the following fields in the following order:

First Name
Last Name
Date of Birth
Gender
Coverage Start Date
Address
City
State
Zip
Phone
Email

Then, simply complete the following form and attach your spreadsheet. Once you have uploaded your file, Teladoc will email each member registration information.
