Termination: Loss of Coverage
Consistent with other benefits, deductions will be terminated on, or before, Date of Termination to avoid over-collection of premiums. Actual coverage ends 30 days after the last payroll deduction date. Premium deduction and reimbursement cease on the next pay cycle following notification. Benefits are not prorated.
