Short Term Disability Insurance (STDI) is intended to cover you for up to 24 weeks following a serious illness or injury; whereas Long Term Disability Insurance (LTDI) is designed to maintain income replacement if your condition keeps you out of work past the end of your short term disability benefit period.

 

ELIGIBILITY AND ENROLLMENT
All full time employees (30+ hours a week) are eligible.
Coverage is based on Annual Compensation.

As a new hire, you MUST submit the enrollment form showing your Acceptance or Waiver of the coverage being offered within 30 days of your hire date.  If you apply for coverage, your coverage will begin on the first day of the calendar month that follows your 30 day eligibility period.

Please note that if you do not choose to enroll in a Disability Insurance plan, you must still submit the enrollment form, waiving participation.  If you do not submit your application within 30 days of being hired, you will be considered a “late entrant".  You will then be required to complete an additional Evidence of Insurability Form for consideration by the underwriters. FOR STDI ONLY, you may also wait until the next open enrollment period to obtain coverage.

FOR THOSE WHO ELECT COVERAGE
On completion of your application we will email you the Certificate Validation Form issued by the carrier (awaiting carrier confirmation.) For your records we also suggest you print the following two documents, STD Policy Certificate and/or LTD Policy Certificate

ANNUAL OPEN ENROLLMENT
Coverage is based on actual annual compensation figures, these will be reviewed and updated by Outstaffing. on the policy anniversary date. Open enrollment is 30 days prior to the anniversary date and ONLY APPLIES TO STDI.

MAKING CHANGES
You may cancel your coverage at any time by sending Outstaffing a signed directive (available in the Forms Library). However, if your status changes from full time to part time you will no longer be eligible to participate and your coverage will be terminated.

PORTABILITY (STDI ONLY)
If your employment is terminated, you may be able to continue coverage at your own expense. Please review your option described in the above STDI - Summary of Benefits Coverage.
