Continuation of Dental/Vision Benefits

You are eligible to continue your coverage through COBRA for Dental and/or Vision.  You will receive information concerning your COBRA eligibility from our third party administrator, UnitedHealthcare Benefit Services.  They will handle all aspects of administration and billing for COBRA for you.  If you have any questions about COBRA or need to update your contact information with UHC Benefit Services, you can call them at 1-877-797-7475.
