Life Insurance Portability

You have the option to continue your coverage directly with Mutual of Omaha Life Insurance.  You will receive an email from us with your portability application attached.  If you do not receive the email, please give us a call and we will resend it either by email or regular mail.

To continue your coverage, simply complete the application and mail it to Mutual of Omaha with your first month’s premium within 31 days of your termination.  They will administer your policy and manage your premiums.  You may contact Mutual of Omaha at 1-800-769-7159.
